UConn-Contracted Service Requests: Facility Operations

UConn Contracted Service Request Form

Use form to initiate requests for University-contracted On-Call Program "GC and Trade Services" or "Professional Services", or initiate Professional Services Selection (Open) Program requests.

Project Number formating criteria are:
#### = Proposed (unfunded) project
3##### = >$100K projects irrespective of funding type and those requiring Capitalization; fiscally administered by Central Accounting and added to Unifier
FO5##### = >$100K projects cost administered by Facilities Operations; those not requiring Capitalization or either Bond or Grant funding
FAC#### = <$100K projects, typically funded by (departmental) Ledger 2 or Ledger 4 accounts or are departmentally funded Maintenance projects
IMF#### = <$100K Projects funded by Facilities Operation’s allocation of the Infrastructure Maintenance (Student) Fee
TL-#### = Academic Renovation (Task Labor) projects; may also be assigned a 3-number in Unifier
